We provide care for patients living in and around Longridge, Inglewhite, Chipping, Whittingham, Goosnargh, Grimsargh, Hurst Green, Ribchester, and Broughton.

If you are unsure whether your address is within our practice boundary, please contact our reception team who will be happy to check for you.


Join the Practice

We are delighted to welcome new patients to Berry Lane Medical Centre.

We offer a flexible service, with appointments available face-to-face and by telephone, six days a week.
Please note: we are unable to register patients who live outside our practice boundary.

You can register with us in one of the following ways:

It is important that you:

  • Enter your current address correctly

  • Provide your previous address and previous GP details

This information helps ensure your registration is accepted by the health authority and allows your medical records to be transferred electronically.


Proof of ID, Address, or Immigration Status

Anyone can register with a GP – you do not need proof of ID, address, or immigration status.
We may ask to see these documents to confirm your details, but we will always register you even if you cannot provide them.


Register at the Practice

You can also register in person during our opening hours (Monday to Saturday).
Please speak to one of our receptionists, who will be happy to assist.

To avoid delays, please have the following information ready:

  • Full name (including any previous names)

  • Date and place of birth

  • Previous address

  • New address and postcode

  • Name and address of your previous GP

  • NHS number, if known (this is not the same as your National Insurance number – you can find your NHS number here)

 

Accessing someone else’s information

As a family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us,we must have pernission from both patients for this. 

To requests proxy access, please collect a proxy access form from reception

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
